4K vs HD Endoscopy: Real Differences and When the Technological Leap Is Worth It

Every time a new technology emerges, every endoscopy service director asks the same question: is it a real advancement or just marketing? The honest answer about 4K in endoscopy is more nuanced than the one provided by manufacturers' advertisements: there are real differences, but they are not the same for all procedures or all services.

1. What 4K Means in an Endoscope

📺
HD (1920×1080p) — EVIS EXERA III / CV-190
2,073,600 total pixels in the final image. Sufficient to clearly view 0.5 mm structures on a 27" monitor. The standard of excellence in endoscopy from 2012–2022. With NBI, it detects 85–90% of small flat adenomas in expert hands.
→ 1080p · 2M pixels · NBI · 2022 reference standard
✨
4K (3840×2160p) — EVIS X1 / CV-1500
8,294,400 total pixels — 4 times the resolution of HD. The EVIS X1's next-generation CCD sensor combines 4K resolution with the TXI (Texture and Color Enhancement Imaging) algorithm, which simultaneously enhances surface texture and mucosal color contrast. On a 31" 4K monitor, the level of detail visibly surpasses HD.
→ 4K · 8M pixels · TXI + NBI · 31" monitor required

2. Real Clinical Differences — Where 4K Matters

Application Does 4K Matter? Why
Detection of flat adenomas (Paris 0–IIb) Yes — significant difference Hayashi 2024: flat adenoma miss rate of 14% (HD) vs 8% (4K+TXI). The largest documented difference.
Diagnosis of Barrett's esophagus with dysplasia Yes — real difference The texture of dysplastic vs non-dysplastic mucosa is better distinguished in 4K. TXI enhances the villous/cryptal pattern.
Early gastric cancer (type 0-IIb) Yes — for trained endoscopists Flat lesions are difficult in HD — in 4K with TXI, the texture difference is magnified.
Screening colonoscopy (pedunculated polyps >1 cm) Minor difference Large polyps are clearly visible in HD — the 4K difference is smaller for bulky lesions.
Routine diagnostic gastroscopy (H. pylori, GERD) Minimal difference For H. pylori and uncomplicated GERD, HD is sufficient — the 4K difference does not change the diagnosis.
Endoscopic hemostasis (bleeding ulcer) No difference Hemostasis does not depend on image resolution but on technique and accessories.

3. The Hidden Requirements of 4K — What Manufacturers Do Not Highlight

4K only works well with at least a 31" 4K monitor — and that comes at an additional cost The EVIS X1 generates a 4K image, but that 4K image is only viewed in 4K if it is projected on at least a 31" certified 4K medical monitor. If connected to a 27" HD monitor (the most common in endoscopy rooms in Colombia), the signal is downscaled to HD and the resolution difference completely disappears. The complete package to obtain a real 4K image includes: CV-1500 + EVIS X1 endoscopes + 31" 4K WM-NX200 monitor + 4K-compatible recording system. The total cost of the complete 4K package vs the EXERA III HD package can be 40–60% higher in Colombia. The correct question is not "Is 4K better?" but "Do my patients and my volume of advanced diagnosis justify that extra cost?".
Is the EVIS EXERA III with NBI still a good investment in 2026?
Absolutely. The EVIS EXERA III (CV-190) with NBI remains an excellent system in 2026 — it is not obsolete technology. For the vast majority of digestive endoscopy procedures in Colombia (diagnostic gastroscopy, screening colonoscopy, standard therapeutic endoscopy), the EXERA III HD with NBI offers excellent diagnostic image quality. The leap to the EVIS X1 4K makes economic sense for reference centers with a high volume of oncological endoscopy (gastric cancer screening, Barrett's surveillance, high-demand CCR screening program) where the improvement in the detection of difficult flat lesions justifies the system's extra cost. For a medium-complexity endoscopy clinic with standard volume, the EXERA III remains the correct cost-effective choice.
