Musculoskeletal ultrasound has gone from being a tool for specialized radiologists to being part of the daily practice of rheumatologists, orthopedic surgeons, and physiatrists in Colombia. The reason is simple: no other modality allows you to see in real time how a tendon moves, exactly where the inflammation of a tendon sheath is, or if a bursitis has internal septa — all at the time of the consultation, without waiting for MRI appointments.

1. Why MSK ultrasound competes with MRI for superficial structures

0.1 mm
Spatial resolution of the 15 MHz linear ultrasound machine — outperforms MRI for superficial tendons
Real time
Dynamic evaluation of moving tendons — impossible with static MRI
No radiation
Neither ionizing (X-ray/CT) nor magnetic field (MRI) — no implant contraindications
Procedure guide
Injections, aspirations, and biopsies guided in real time — greater precision

2. Main indications of MSK ultrasound

💪
Tendons and tendon sheaths
Tendinopathies: degeneration of the patellar tendon, Achilles tendon, rotator cuff, hand extensor tendons — thickening, loss of normal fibrillarity, neovascularization on power Doppler. Ruptures: partial or complete rotator cuff rupture, Achilles tendon rupture — hypoechoic gap in the tendon, fiber discontinuity. Tenosynovitis: fluid in the tendon sheath, thickened synovium with positive Doppler in active rheumatoid arthritis.
→ Rotator cuff · Achilles · Patellar tendon · RA tenosynovitis
🦵
Joints — synovitis and effusion
Joint effusion: anechoic or hypoechoic fluid in the joint recess — even small volumes (<1 mL) that clinical examination does not detect. Synovitis: hypervascularized synovial thickening (positive color Doppler) in rheumatoid arthritis, psoriatic arthritis, and other spondyloarthropathies. MSK ultrasound detects active synovitis before erosive changes appear on X-ray — key for early treatment.

Procedure guide
Joint injections (hip, shoulder, ankle, small joints of the hand — much greater precision than blind injection). Bursa aspirations (subacromial bursa, olecranon bursa). Soft tissue mass biopsy. Barbotage of rotator cuff calcifications. Studies show that ultrasound-guided injection has 95–99% accuracy vs 60–70% blind.
→ Joint injection · Bursa aspiration · Barbotage · 95–99% accuracy
🦶
Peripheral nerves
Carpal tunnel syndrome — thickening of the median nerve at the level of the retinaculum (area >10 mm²). Morton's neuroma between the 3rd and 4th metatarsals. Ulnar neuropathy in Guyon's canal. Evaluation of traumatic nerve injuries. Ultrasound has the advantage of being able to follow the nerve along its entire course — something MRI cannot do with the same ease.

3. What ultrasound machine do you need for MSK — frequency is everything

Transducer Frequency For what MSK structures Limitation
High-frequency linear 10–15 MHz Superficial tendons (rotator cuff, Achilles, hand tendons) · Peripheral nerves · Small joints of the hand · Superficial bursae Low penetration — not suitable for hip or deep shoulder in obese patients
Medium-frequency linear 7–10 MHz Medium-sized joints (knee, shoulder in thin patients) · Deep muscle Lower resolution than 15 MHz for very superficial structures
Low-frequency convex 3.5–5 MHz Hip (injection guide, effusion) · Shoulder in patients with thick adipose tissue Low detail resolution — only for deep structures
The 15 MHz linear transducer is essential for quality MSK ultrasound An ultrasound machine with only an abdominal convex transducer is not suitable for MSK ultrasound. The high-frequency linear (10–15 MHz) is the specific transducer for MSK — and it is one of the most expensive ones (USD 4,000–12,000 for the transducer alone). If MSK ultrasound is your main practice, the cost of the transducer must be included in the equipment budget. The VINNO G60 with L741 linear transducer (4–15 MHz) and the Mindray DC-60 with L14-6WU are the most installed options in rheumatology practices in Colombia 2026.

📋 Frequently asked questions also answered by this article

What is musculoskeletal ultrasound and what is it used for?
Is MSK ultrasound better than MRI for tendons?
What ultrasound machine do I need for rheumatology and MSK?
How much does a musculoskeletal ultrasound cost in Colombia?
Does ultrasound detect rotator cuff rupture?
Can I perform ultrasound-guided joint injections in my office?
Can I do MSK ultrasound with the same ultrasound machine I use for abdomen?
It depends on the ultrasound machine and the transducers you have. If your ultrasound machine has only a 3.5–5 MHz convex transducer: it is not suitable for MSK — the frequency is too low and the resolution insufficient to see tendons and nerves. If you have an ultrasound machine with a 10–15 MHz linear transducer (high frequency): yes, you can do quality MSK ultrasound. Many gynecological and internal medicine ultrasound machines do not include the high-frequency linear transducer in the basic package — it is an additional transducer that is purchased separately. Ask GSMED which transducers are available for your current ultrasound machine model and if they are compatible.
