The X-ray machine is the most ubiquitous medical imaging equipment in Colombia — it is in hospitals at all levels, clinics, doctor's offices, and rural health centers. But not all X-ray machines are the same: there is a huge difference in quality, speed, and operating cost between an analog system from the 1990s and a 2026 direct digital radiography (DR) system. This guide explains what you need to know to choose well.

1. Anatomy of a fixed-room X-ray machine

A fixed-room X-ray machine has three major subsystems that work together to generate a radiograph:

⚡
The generator
Converts the electrical current from the grid into the high voltage (kV) needed to accelerate electrons in the X-ray tube. Modern generators are high frequency (HF) — smaller, more stable, and with a lower dose to the patient than old low frequency generators. Typical power: 32–80 kW. The generator can be floor-mounted, ceiling-mounted, or integrated into the control panel.
🔦
The X-ray tube
The component that generates the X-rays. It contains the cathode (filament that emits electrons) and the rotating tungsten anode (where the electrons impact and generate the X-rays). The tube has a finite lifespan — usually expressed in accumulated Heat Units (HU). Modern tubes support loads of 600,000–1,000,000 HU. It is the most expensive component to replace.
📡
The detector (DR flat panel)
Captures the X-rays that pass through the patient and converts them into a digital image. Modern detectors are amorphous silicon (a-Si) or amorphous selenium (a-Se) flat panels. Standard sizes: 35×43 cm (chest/abdomen) and 24×30 cm (extremities). Detector resolution is measured in lp/mm (line pairs per millimeter) — typically 2.5–4.5 lp/mm in general radiology.
32–80 kW
High-frequency generator power in general radiology equipment
43×43 cm
Maximum flat panel size — covers the entire chest in a single exposure
3 sec
Image appearance time in direct DR systems — without processing
DICOM
Connectivity standard — integrates the equipment with PACS and RIS scheduling system

2. Types of fixed-room X-ray machines

Tube + DR detector fixed in the table
Traumatology · Emergency room · Orthopedics
Type Configuration Advantages Ideal for
Ceiling-mounted tube system Ceiling-mounted rail-suspended tube + wall-mounted detector + examination table Greater tube range of motion · Accessibility for wheelchair patients · Occupies the entire room volume General hospital radiology · High volume · Chest + extremities + spine
Floor stand system Tube on floor stand + wall-mounted or table-mounted detector No ceiling work required · Faster installation · Lower initial cost Medium-sized clinics · Installation in rooms without specialized ceilings
Tilting table system (conventional fluoroscopy) Tube under the tilting table + image intensifier or flat panel Allows dynamic fluoroscopy · Contrast studies (transit, cystography) Radiology with contrast studies · Hospitals with gastroenterology
Direct digital system with fixed detector More ergonomic system for bedridden patients · Detector always in position

3. Brands available in Colombia and key differences

🇩🇪
Agfa HealthCare (DX-D 300 / DX-D 500)
The leading European brand in direct digital radiography. The DX-D 300 is the entry-level direct DR system — 35×43 cm flat panel with 2.5 lp/mm resolution. The DX-D 500 is the premium system with a 43×43 cm detector and 3.4 lp/mm resolution for high-demand radiology. MUSICA software for automatic image optimization. Strong presence in high-complexity Colombian hospitals.
→ Premium image quality · MUSICA software · Present in high-complexity Colombia
🇯🇵
Fujifilm (FDR D-EVO III)
The Fujifilm DR system with ISS (Irradiation Side Sampling) detector that captures X-rays on the input face of the detector instead of the output face — which improves image resolution especially in fine structures such as bone trabeculae. Standard 35×43 cm flat panel. Virtual Grid software to reduce the need for an anti-scatter grid, simplifying procedures.
→ ISS detector · Virtual Grid · Bone trabeculae resolution
🇺🇸
Carestream (DRX-Excel)
Modular system with a 35×43 cm wireless DR detector (wireless cassette) that can be used in both a fixed room and at the bedside with the same detector. Cost-effective solution for institutions that need to cover both fixed-room and portable radiography with a single detector. Good presence in medium-sized Colombian clinics.
→ Wireless detector · Dual fixed/portable use · Cost-effective
🇯🇵
Canon Medical (CXDI series)
Canon CXDI flat panels are reference detectors in image quality — especially for mammography and fine musculoskeletal radiology. High-end systems with a high price. Less presence in Colombia than Agfa or Fujifilm, but available for high-complexity institutions that prioritize maximum image quality.
→ High-end · Reference image quality · Lower presence in Colombia

4. Mandatory authorization in Colombia — what you cannot skip

Regulatory requirements to operate an X-ray machine in Colombia (1) INVIMA registration of the equipment — verify that the specific model has a valid registration; (2) ANLA operating license — National Authority for Environmental Licenses, to operate ionizing radiation sources; (3) Medical physicist report — certifies the room shielding (walls, lead door, lead window), the technical staff dosimetry, and the radiological protection protocol; (4) Authorization of the radiology service before the departmental Health Secretariat (Ministry of Health Resolution 2003/2014). The complete process takes 2–4 months. Without these 4 steps, operating the equipment is illegal in Colombia.

5. Frequently asked questions

How much does a digital X-ray machine cost in Colombia?
The price varies depending on the system type and brand. A new complete fixed-room DR system (generator + tube + 35×43 cm flat panel + software) can cost between USD 35,000 and USD 90,000. A high-end system with a 43×43 cm flat panel and greater power can exceed USD 100,000. To this, you must add: room shielding (USD 5,000–20,000 depending on size), medical physicist fees (USD 2,000–5,000), and specialized electrical installation. Contact GSMED for a specific quote according to your needs.
How long does an X-ray tube last?
The lifespan of an X-ray tube depends on the number of exposures, the load per exposure (kV and mAs), and the cooling time between exposures. In practical terms, in a medium-demand radiology service (20–40 radiographs daily), a modern tube can last 5 to 10 years. Manufacturers express the tube's lifespan in accumulated Heat Units (HU) — typically 600,000–1,200,000 HU. Modern systems have accumulated HU counters to plan the preventive replacement of the tube before it fails unexpectedly.
