Urinalysis is one of the most requested laboratory tests in Colombia — its low cost and high diagnostic yield make it the first study for investigating urinary tract infections, kidney diseases, diabetes, and urological conditions. Modern equipment automates physicochemical analysis with dipsticks and the counting of formed elements in the sediment in an integrated system.

1. The Three Phases of Complete Urinalysis

🌈
1. Physical Analysis
Evaluation of color (yellow, amber, red, cloudy), appearance (clear vs. cloudy), and urine specific gravity (using a refractometer or dipstick: 1.005–1.030). Cloudiness may indicate pyuria (infection), crystalluria, or chyluria. Very concentrated urine (specific gravity >1.025) suggests dehydration; very dilute urine (<1.005) may indicate diabetes insipidus.
🧪
2. Chemical Analysis (Dipsticks)
Multiparameter dipsticks simultaneously analyze 10–12 parameters in seconds: pH, protein, glucose, ketones, bilirubin, urobilinogen, blood/hemoglobin, leukocytes (leukocyte esterase), nitrites (gram-negative bacteria), creatinine, and specific gravity. Urinalysis analyzers read the dipsticks using reflectometry — more reproducible than subjective visual reading.
🔬
3. Microscopic Sediment
Analysis of centrifuged urine to identify and quantify: leukocytes, erythrocytes, casts (hyaline, granular, erythrocyte, leukocyte), epithelial cells, bacteria, crystals, yeast, and sperm. Automated sediment analyzers (Sysmex UX-2000, iQ200) capture images of the particles and classify them using AI.

3. Clinical Interpretation of the Most Common Findings

Urinalysis Finding Most Likely Clinical Significance Action
Positive nitrites + leukocyte esterase Bacterial urinary tract infection (UTI) Urine culture + antibiogram before starting antibiotics
Urine glucose (glucosuria) Uncontrolled diabetes mellitus or kidney diabetes Fasting blood glucose + HbA1c
Protein ++/+++ (proteinuria) Nephropathy (diabetic, hypertensive, glomerulonephritis) 24-hour urine protein or protein/creatinine ratio
Microscopic hematuria (>5 erythrocytes/field) Renal lithiasis, urinary tract infection, urothelial tumor, glomerulonephritis Renal ultrasound + ureteroscopy if hematuria without infection
Erythrocyte casts Active glomerulonephritis Refer to nephrology — kidney biopsy may be necessary
Positive ketones Diabetic ketoacidosis or prolonged fasting Urgent blood glucose — if >250 mg/dL with ketones: emergency department
Calcium oxalate crystals Calcium oxalate urolithiasis Renal ultrasound + hydration + dietary oxalate control
Why Automated Sediment Analysis Improves Diagnosis Manual microscopic sediment analysis has significant inter-observer variability — an experienced bacteriologist and a recent graduate may report different results from the same sample. Automated imaging sediment systems (Sysmex UX-2000, iQ200 SPRINT) capture digital images of each particle, classify them using artificial intelligence, and allow the bacteriologist to review only the particles flagged as abnormal or difficult to classify on the screen. This reduces processing time, improves reproducibility, and allows relevant findings to be documented photographically in the medical record.
Can a dipstick replace microscopic sediment analysis?
Not completely. The dipstick chemically and indirectly detects the presence of leukocytes (via leukocyte esterase) and erythrocytes (via hemoglobin peroxidase). Microscopic sediment analysis visualizes them directly and can also identify casts, crystals, atypical epithelial cells, and bacteria — information the dipstick cannot provide. CLSI GP16-A3 guidelines recommend performing microscopic sediment analysis on any urine sample with positive dipstick results (protein, blood, leukocytes, nitrites) and as a quality control measure to validate automated sediment results.
